As per the title deed
The signature of all owners, as indicated on the evaluation roll for the property in question, is required. One person may be designated, in writing, to represent all the owners for the present request (see section "Identification of the applicant").
If the registered owner is a Company (legal entity) and the designated representative is not the major shareholder, attach a certified copy of the resolution from the Administrative Council authorizing this person to sign the permit application.
For new owners who are not currently registered on the evaluation roll, please attach a copy of your deed of sale.
You may also attach a signed proxy from the owner authorizing the applicant to sign or receive information issued by the Municipality of Bolton-Est on their behalf.

A location plan done by a professional, to scale, identifying all existing and proposed wells, serviced buildings, property lines, watercourses, flood risk zone and 0-20 yrs and 20-100 yrs marks if applicable

If it involves taking lake water, provide a plan localising the pipe on the shore and a side view plan showing the site slope, the depth of the pipe, type of fill and a desritpion of soil stabilisation , which shall prevent any brown water to reach the lake during and after the work is done
If ground water catchment is involved (other than lake or watercourse catchment), you are also requested to present the location of any watertight and none watertight waste water systems and the distance between those system parts and the ground water catchment installation proposed within a 30m radius, plus the location of the following elements also within a 30m radius of the ground water catchment installation: composting area, exercise yard, livestock operation, manure heaps or pit, a cultivated field, a pasture or a cemetary
When located in the green zone, also show the location of the surface area granted by the Commission de Protection du Territoire Agricole du Québec (CPTAQ)

The fees associated to this permit application are $25.
Copy of the report complying with article 21 of Provincial by-law Q-2, r. 35-2 shall be presented to the municipality within 30 days of the completion of work.
